Preparing for your first visit

Prior to your appointment, please complete and submit the patient forms linked below and provide a copy of your insurance card front and back. Please also bring:

  • Previous consultation reports and school (IEP) reports

  • Outside records of any neuroimaging reports (w/digital CD if available), blood work, or neuroimaging reports
