For prescription refills, please contact our office directly at least three business days in advance, if possible. Refill requests submitted through pharmacies are frequently misdirected and may be automatically denied. Please click here for a medication refill.

For medical emergencies requiring immediate medical attention, please call 9-1-1. For other urgent matters, there is a pediatric neurologist available 24 hours/day by calling the main office and following our voice prompts.

Please fill in your CHILD'S NAME and DATE OF BIRTH along with all the portions designated to be filled out by Parent PRIOR to submitting your form to us.

Your form will be returned to you in 3 to 5 business days. A fee will be due upon return of form.

Yes, we offer FOLLOW UP appointment telemedicine options. However, to provide the best care for your child, all NEW patient evaluations are done in-person.

If you would like results before your follow-up appointment, please call the office 3-5 business days after tests were completed.
