We will prescribe enough medication to last until the recommended follow-up appointment. Should you need a refill sooner or for certain controlled medications, please request refill 3 business days before you run out of medication. Please click here for a medication refill.

For medical emergencies requiring immediate medical attention, please call 9-1-1. For other urgent matters, there is a pediatric neurologist available 24 hours/day by calling the main office and following our voice prompts.

Please fill in your CHILD'S NAME and DATE OF BIRTH along with all the portions designated to be filled out by Parent PRIOR to submitting your form to us.

Your form will be returned to you in 3 to 5 business days. A fee will be due upon return of form.

Yes, we offer FOLLOW UP appointment telemedicine options. However, to provide the best care for your child, all NEW patient evaluations are done in-person.
